Your Choices about Our Collection, Use and Disclosure of Your Information

We strive to provide you with choices regarding the personal information you provide to us. This section describes mechanisms we provide for you to control certain uses and disclosures of your information.

  • Tracking Technologies. You can set your browser to refuse all or some browser cookies, or to alert you when cookies are being sent. If you disable or refuse cookies or block the use of other tracking technologies, some parts of the App may then be inaccessible or not function properly.

Third-Party Interfacing

We do not sell, market or otherwise transfer your personal information to third-parties. The App does, however, interface with certain third-party provided technology that allows us to send you automatic notifications (“push notifications”) to alert you to messages from your care providers, appointments, etc. based on demographics, App usage and other relevant factors. The third-party provider WILL NOT have access to your personal information.

Data Security

We have implemented measures designed to secure your personal information from accidental loss and from unauthorized access, use, alteration and disclosure, and have assured that our cloud storage provider will provide acceptable levels of security.

The safety and security of your information also depends on you. Where we have given you (or where you have chosen) a password for access to certain parts of our App, you are responsible for keeping this password confidential. We ask you not to share your password with anyone.

Pursuant to various state and federal laws, we may be required to send you notice of security breaches or suspected security breaches that impact your personal information and PHI. We will send you security breach notices to the email address contained in your account information. Please note: Many email systems have SPAM filters. If you have one in place, you should check with your system administrator or the available instructions to confirm that emails from us are not blocked by the filter (e.g., by confirming that the Service domain name (http://www.ohmd.com) is a permitted domain name).

Changes to Our Privacy Policy

We may update our privacy policy from time to time. If we make material changes to how we treat our users’ personal information, we will post the new privacy policy on this page. The date the privacy policy was last revised is identified at the top of the page. You are responsible for ensuring we have an up-to-date active and deliverable e-mail address for you and for periodically visiting this privacy policy to check for any changes.
